Seeking Customer-Specific Demo Form Ideas for Upsell/Cross-Sell

Does anyone have a demo form that's specific to customers? We're working toward building a separate "request a demo" form for upsell/cross sell and looking for some inspo.

yes will share 2. where are you thinking of using this? something we've seen working well is giving info in app or via email and then having them click a button (that is tracked) that says I'm Interested and then that click is the flow instead of them going to a page and giving info that we already have from them. (not sure if that makes sense out loud lol) but essentially if we have an email telling them all about the paid feature then we say something like If you'd like a member of the team to give you more information about this feature, click the I'm Interested button below and your dedicated team member will reach out' and when they click the link it goes to a thank you page
