We use Calendly for this at our events, works well on phones/iPad and can even have a QR code setup too.

Not sure if Descript is what you're looking for but I've used it in the past to transcribe audio to capture the text to add captions and could also export the text as well I believe.

Totally get the intention, But since “agrees to consider” isn’t binding, it doesn’t really help with planning advocacy, and without any timeline, it’s not something that is easy to trigger or act on in the future. From what I've seen including something like this as part of your onboarding / CSM flow can be more effective.

You could also just use a google form with similar information to the downloadable template. Edit: I was too slow!

Previously sales teams that I have worked with have used it in negotiations. I.e if a customer or soon-to-be customer is asking for a discount or changes to an agreement, as a return we will ask if they would participate in a webinar, case study, x reference call(s) etc.
